What to do in preparation for your first visit:

View the hours and location section below for directions to our office.
Please bring your driver's license (or current identification card) and any insurance cards with you when you visit our office.
Please bring in any studies, such as x-rays and MRI scans that you may already have, which are related to the reason for your visit.
Please print the patient forms listed below and bring the completed forms to your appointment:

The Notice of Privacy Practices form below is for your review. You do not need to print it or bring it to your appointment. We do ask that you print and complete the above Receipt of Notice of Privacy Practices Form (above) to acknowledge the receipt of the notice of privacy practices.

Hours and Location

Office hours are by appointment only.

The Georgia Osteopathic Care Center is inside the main building at PCOM Georgia located at 625 Old Peachtree Road NW in Suwanee, Georgia. Free, reserved patient parking is located in front of the building.
